Dear Wood County Community Member,
Help me to help you! One of NAMI's roles in the community is to advocate for individuals living with mental illness and the family members affected. It is hard to appropriately advocate without hearing from community members, YOU!
If you could take 5-10 minutes to complete this open-ended questionnaire, I would greatly appreciate it.
The information provided will be compiled into a survey summary and submitted to the Alcohol, Drug Abuse, and Mental Health Services Board (ADAMHS) of Wood County. The Wood County ADAMHS Board will use the survey results in developing programming in the community, implementing change, prioritizing community needs, and establishing their own strategic plan.
In the survey summary, I will be grouping information into common themes. Because the summary is compiled and submitted by NAMI and there is a requirement to be brief, not all comments will have the opportunity to be included.
If you have a vision for change and would like your voice to be heard, please use NAMI to be that collaborative loud voice in Wood County.
