Volunteer Physical Requirement Agreement

Thank you for your interest in volunteering with NAMI Wood County.  In order to ensure the safety and success of our programs, we may require volunteers to do physical tasks in the office such as sweeping floors or moving chairs/tables. Our organization does our best to accommodate for all of our volunteers and have tasks available for all abilities and body types. However, there may be some months where this is not an option.

With this in mind, please let us know of any physical limitations. We will email you within 1-2 days of your registered date if we do not have any tasks available to accommodate your abilities.

I understand that I may not be able to volunteer with NAMI Wood County due to limited tasks available to accommodate my abilities.
